For your convenience, we have included downloadable documents that will expedite various building service requests. Hard copies of all forms are available from the Management Office as well.
Please fill out the following forms and return via email to apm@titaninv.com. If you have any questions regarding these forms please call Karla Laurent, Senior Portfolio Manager at 303.321.5070.
- Access Card Request Form - to be completed every time there is a change in staff for your suite.
- After-Hours Contacts Information Request - to be completed anytime the emergency contact for your suite changes.
- Building Rules & Regulations Form
- COVID-19 Waiver - must be included with Fitness Waiver and for use of the Conference and Training Rooms.
- Emergency Evacuation Response Form - Please update and forward to the management office as changes in personnel occur.
- Health Club Waiver - to be completed anytime an individual wants access to the fitness room. COVID-19 Waiver must be included with Health Club Waiver for access to Fitness Center and Locker Rooms.
- Holiday Schedule - to notify building management when your business will be opened or closed for the mentioned holidays.
- Moving Policy and Vendor / Contractor Work Approvals Form
- Parking Regulations Form
- Persons with Disabilities Form - Please update as changes in personnel or physical conditions occur; forward updated copy to the management office.
- Signage Request Form - to be filled out anytime signage needs to be ordered for the main lobby directory and/or for your suite entry. Print the suite name as it should appear on the signage and fill in the suite number. Please be sure to include commas and periods if required. Return or fax the form to the Management Office when complete.
- Tenant Information Request Form - to be completed anytime the contacts for your suite change.
